All Medicare patients are required to have a referral from their physician.

Some private insurance companies require a prescription as well so having a Rx from your doctor is always best.

However, under "direct access" you can attend physical therapy for 30 days without a Rx, with the exception of Medicare and other gouvernamental insurance plans. So, give us a call for details.

We accept most insurance plans. If your plan is not listed below, please call our Scheduling Department at (352) 376-7505 or toll free (888) 653-6651 as we are always adding new insurance plans.

  • AARP
  • Aetna
  • Auto
  • Avmed
  • Blue Cross/Blue Shield
  • Humana
  • Medicare
  • Tricare Standard
  • United Health Care
  • Worker’s Comp
  • A prescription for physical therapy from your referring physician, ARNP, or PA.
  • Insurance card (we will make a copy of your card).
  • In case of an automobile accident or worker’s compensation claim, also bring contact information of your case manager or insurance adjuster, and your claim number.
  • Please arrive 15-20 minutes early to complete paperwork.

Items you will need for Aquatic Therapy:

  • Towel
  • Bathing suit or wet suit
  • Soap and/or Shampoo and items needed for showering
  • A plastic bag (for wet clothes)
  • Skid free shoes or boat shoes in the pool area.
  • Lock (if you need to secure your valuables)

Bare in mind that all patients MUST shower before entering the pool. You don't have to get your head/hair wet.

Casual loose fitting clothing, such as shorts, sweats, and workout shoes.

Please be aware that the benefits we verify will be provided to us by your insurance company and are not a guarantee of payment by your insurance company. Payments we receive for the services provided to you, may at times be processed differently by your insurance company than the benefits provided to us by your insurance company.
